Social prescribing

In going about your day-to-day job, you may come across a patient who’s wellbeing is being impacted by non-medical challenges such as social isolation, bereavement, housing problems or financial difficulties.

NWAS crews can help these patients by referring them to the social prescribing services run by their GP practice or council. A social prescribing link worker will then work with the patient to develop a support plan and help them to connect with the most suitable activities, groups, and services in their community based on their interests and needs.

In these videos, we will be following a patient’s journey as they are collected from their home by a PTS crew. The crew notice that the patient may benefit from a referral to social prescribing, and discuss this with the patient.
